Forms, Worksheets, and IRS Publications

The links on the left are downloadable forms and worksheets that you can use to prepare for your tax preparation session. The worksheets also serve as a handy checklist for tax deductions if you are itemizing and deductible expenses for your small business Schedule C.

If you can't download and print out these forms in advance, don't worry. All forms will be available at the tax prep sites. However, you may still want to review these forms, particularly the interview form so you can bring in your supporting documentation and be ready to answer the questions on the form.

  • IRS Form 13614-C Intake Interview - Quality Review Sheet -- Use this form if you have scheduled an appointment at the Placerville Senior Center. Please print out this form, complete the interview and consent sections, and bring this with you for your tax preparation appointment.
  • IRS Form 13614-C Intake Interview - Quality Review Sheet AARP Tax-Aide Booklet -- Same as above, but with the additional Tax-Aide consent forms and interview questions. Use this form if you have scheduled an appointment at an AARP Tax-Aide site. Please print out this form, complete the interview and consent sections, and bring this with you for your tax preparation appointment.
  • Itemized Deduction Worksheet -- If you are itemizing, please download and complete this form. Total all medical expenses, mortgage interest expenses, property taxes, etc. in the space under the “Total Amount” column. Bring your documentation with you in case there are any questions regarding the eligibility of your deductions.
  • Self-Employed (Schedule C) Worksheet -- If you are self-employed or run a small business, this worksheet contains most of the information we need to complete your Schedule C. Total all amounts in the expense categories listed on the worksheet. Please bring in your expense documentation in case there are any questions about your expenses.

IRS Publications

All of the publications below are available on the Internal Revenue Service Website. We have included a set of the most popular publications here.

  • Publication 17, Your Federal Income Tax (For Individuals) -- Publication 17, Your Federal Income Tax (For Individuals), covers the general rules for filing a federal income tax return. It supplements the information contained in your tax form instructions. It explains the tax law to make sure you pay only the tax you owe and no more.
  • Publication 334, Tax Guide for Small Business -- Publication 334, Tax Guide for Small Business, provides general information about the federal tax laws that apply to self-employed persons or statutory employees.
  • Publication 502, Medical and Dental Expenses -- Publication 502 explains the itemized deduction for medical and dental expenses that you claim on Schedule A (Form 1040) and the health coverage tax credit that you claim on Form 8885.
  • Publication 503, Child and Dependent Care Expenses -- Publication 503, Child and Dependent Care Expenses, explains the tests you must meet to claim the credit for child and dependent care expenses. It explains how to figure and claim the credit.
  • Publication 504, Divorced or Separated Individuals -- Publication 504 explains tax rules that apply if you are divorced or separated from your spouse. It covers general information to help you choose which filing status you are entitled to and which benefits you can claim. The publication also discusses issues such as dependents and their status, payments for alimony, child support, and other court-ordered payments, including the special rules that apply to community property states such as California.
  • Publication 530, Tax Information for Homeowners -- This publication explains how you treat items such as settlement and closing costs, real estate taxes, sales taxes, home mortgage interest, and repairs. The following topics are explained:
    • What you can and can't deduct on your tax return.
    • The tax credit you can claim if you received a mortgage credit certificate when you bought your home.
    • Why you should keep track of adjustments to the basis of your home. (Your home's basis is generally what it cost; adjustments include the cost of any improvements you might make.)
    • What records you should keep as proof of the basis and adjusted basis.
  • Publication 554, Tax Guide for Seniors -- Publication 554, Tax Guide for Seniors, provides a general overview of selected topics that are of interest to older tax-payers. The publication will help you determine if you need to file a return and, if so, what items to report on your return. Each topic is discussed only briefly, so you will find references to other free IRS publications that provide more detail on these topics if you need it.
